Apex Wellness is committed to protecting the privacy and security of your information. Because we provide health-related services, we are required by federal and state law—including the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”)—to maintain the privacy and confidentiality of your Protected Health Information (“PHI”).
Protected Health Information (PHI)
PHI includes any information that can identify you and relates to your past, present, or future physical or mental health condition, the provision of health care to you, or payment for health care services. We will not use or disclose your PHI for marketing, research, or any other secondary purposes without your explicit authorization.
Data Security
We use administrative, technical, and physical safeguards designed to protect your information, including encryption of electronic records, secure servers, and access controls. Despite these measures, no system can guarantee 100% security, and you acknowledge this when using our services.
Third-Party Disclosures
We do not sell or rent your personal or health information to third parties. We may share limited information with third-party service providers (e.g., pharmacies, laboratories, payment processors) only to the extent necessary to provide our services. All third-party partners are contractually obligated to safeguard your information and comply with HIPAA standards.
